I/we hereby agree that this payment is for the designated Friday dates only and does not entitle me/us to curl at any other time. Missed Friday sessions are forfeit unless cancelled by the Club due to weather other unforeseen circumstances.

Liability Waiver

I/we hereby agree to waive and to hold harmless the "Smiths Falls Curling & Squash Club "(the "Club"), its officers, employees and volunteers from any claim whatsoever arising from personal injury and/or property damage as a result of use of the "Club" equipment and the premises, except claims arising from the negligence of the "Club", its officers, employees and volunteers.

I/WE further agree to be responsible for any damages incurred to the "Club" equipment and premises during our use and will be responsible for paying to repair any such damages.  It is also understood and agreed that the "Smiths Falls Curling Club" will not be held responsible for property, personal or otherwise, left on the premises.
